What To Bring
On the day of your child's first visit to our office, we recommend arriving a few minutes early. Arriving early will give you time to fill out needed paperwork if you choose to complete it at the clinic. We will also need to scan any documents relevant to your child, such as photo id, insurance cards, guardianship papers, etc. During the first visit, we ask for the primary guardian to bring the child in for their first visit with us. This will give the provider the opportunity to meet you and understand any concerns you may have regarding your child. The provider will also ask questions about past medical history, any surgeries the child has had, and family history.
Our door is always open for new patients or patients returning to our clinic after being away. At Children's Health Center we provide today’s latest treatments with old-fashioned tenderness because your child is special to us! Below you will find the paperwork, what to expect on your first visit, and paperwork you will need to bring for us to best treat your child.
*Please note that the Current Patient Information, Pediatric History, HIPAA & Policy Review must be filled out for your child to be seen in our clinic. If your child has government insurance the HIPAA & Vaccine Eligibility must be filled out also.*
